Observed rooms 101 and 102 with cigarette burns on top sheet. Observed loose hairs and debris on beds and sheets in all rooms and pull out couches (x2). Observed stained pillows x2 in room 103. Observed fitted sheet in room 205 and bed bug cover in room 110 stained. C/A-Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed leaking hole in ceiling of laundry room. C/A- Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Correct by date: April 14, 2026

Observed main floor ice machine with black build up inside shoot and drain. Observed ice machine also leaking. C/A: Ice machines and equipment shall be clean, sanitized, and maintained to prevent accumulation of build-up

Observed staining of mattress pads and pillows in room 102. Observed staining of fitted sheets in rooms 104 and 105. Observed staining on mattress protectors in room 105 and 211. Observed staining on mattress pad of rooms 203 and 211. C/A: Bed, bedding, and linens shall be clean, sanitized and maintained to prevent staining and pests

Observed a hole in ceiling of laundry room and a hole in wall around shower head in room 104. C/A: Floors, walls, ceilings, windows, doors and all other accessories shall be of sound construction, properly maintained in good repair and shall be kept clean.
