Observed stained pillows in rooms 127 (dead bug inside pillow case), 120 (torn), 211, and 228. Observed stained mattress in rooms 127 and 228. Observed undersized mattress protector on the bed in room 211. Observed no mattress protector on the sofa bed mattresses in rooms 211 and 228.

C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed hole in the wall under the sink in room 120. Observed trash under the bed/behind furniture in rooms 120, 211, and 228

C/A: Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean

Observed stained bedding in the following rooms: 125 and 127. Observed stained pillowcases in room 123.

Observed window curtains in rooms 309, 324, 213 to be heavily soiled and stained. CA: All draperies and curtains must be maintained in good repair, clean, and free of vermin

Observed all rooms to have several large stains and/or rips/holes in bedding. CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and maintained

Observed smooth tub surfaces in rooms 309 and 324. CA: Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair

Observed 2 unlabeled cleaning chemical bottles on first floor in housekeeping cart. Observed trash and used utensils near clean linens on cart as well. CA: Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label
