Observed temperatures above 120F in multiple rooms. Room 439: 127F Room 341: 130F Room 340: 127F CA: Hot water in all guest rooms shall not exceed 120F

PIC could not locate gas-fired equipment inspection. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available

Observed a stained and torn mattress in room 226. Observed staining on pillows and pillow covers in rooms 220, 311, and 434. C/A: Bed, bedding, and linens should be clean, free from stains, and in good condition

Observed build up at base of showers in rooms 226, 220, and 434. C/A: Bathing facilities should be free from build-up and re-caulked to prevent moisture from seeping into the crevices of the shower

Several stains were observed on the mattresses inside rooms 439 and 216. CA Remove the stains and recommend provide a mattress cover or bed bug cover to ensure the mattress remain clean. A stain was observed on the pillow sheet inside room 439. CA Remove the stain and/or replace the sheet. COS The sheet was removed

Several peel spots were observed on the shower floor inside room 216. CA Recommend resurfacing the shower floor. Remove the mold on the shower strip on the floor outside of the shower. CA Recommend recaulking and/or replace the strip inside room 111. The tab on the knob of the sink inside room 320 was missing. CA Replace the tab. Properly patch up and paint the ceiling above the shower. It appears that a leak occurred on the ceiling inside room 320. A broken toilet and wall tiles were observed inside the restroom inside the hallway on the first floor. CA Ensure all of the toilets are operating properly and repair the wall

Mold was observed inside the ice machine on the 4th floor. Replace and or attach the tray and pipe to the bottom of the ice machine

An accumulation of detergent was observed on the sides of the washer machines. Garbage was observed on the floor next the washer machines. CA Laundry rooms must be in good condition and maintained. The dryers were observed not operating. The facility is waiting on part to arrive to repair the dryers. Linens are taken to the laundromat to dry. COS Equipment must be properly operating

Stains were observed on the mattress in room 429. The mattress pad material inside room 248 was observed peeling. CA Replace and clean the mattress

A small crack in the ceiling was observed above the shower inside room 239 from what appears to be water damage. The wood panel on the stall door inside women's restroom on the first floor was observed hanging off the door. CA Properly seal the ceiling and repair the panel on the door

Mold was observed inside the dispenser and bottom tray of the ice machine on the 4th floor. CA Sanitize equipment to prevent contamination

A stain was observed on the mattress inside room 436. CA Replace and or sanitize the mattress to remove the stain

Pink residue was observed around the perimeter of the bathtub inside room 436. CA Recommend sanitizing and re-caulking the bathtub

Mold was observed inside the dispenser of the ice machine on the 3rd floor. CA Sanitize equipment to prevent contamination

Scratches were observed inside the shower in room 340 and inside the toilet in room 437. The seal was observed loose around the bathroom faucet inside room 340. CA Bathrooms must be maintained clean and in good condition

Scratches were observed on the ceiling above the closet inside room 340. CA Physical facilities must maintained and in good repair
