The most recent tourist accommodation inspection report was not posted in public view, nor was it available upon request.

C/A: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed buildup on the wall of the shower in room 215. C/A: Restroom facilities must be clean, sanitized, and free from buildup and residue

Observed black buildup on ice machines on 1st and 4th floor.

C/A: Ice machines should be clean and sanitized according to proper procedures

Observed unlabeled chemicals on housekeeping carts on 2nd and 3rd floor.

C/A: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents

Observed staining on the bed bug cover and pillow covers in room 201, and stains on pillowcases in room 418. Observed ripped mattress covers in rooms 309 and 418. C/A: Bed, bedding, and linens shall be free from stains and in good condition

Observed buildup on the ceilings of the showers in rooms 103 and 331. C/A: Restroom facilities must be clean, sanitized, and free from buildup and residue

Observed build up on ice machines on floors 1, 2, 3, 4, and 5. C/A: Ice machines should be clean and sanitized according to proper procedures

A brown stain on the curtain was observed inside room 104. A light yellow stain was observed on the bed bug cover of the bed closest to the night stand in room 309. What appeared to be ripped sheets were observed on the second bed closest to the window inside room 201. CA Bed, bedding and linens must be clean and in good condition

Mold was observed inside the shower in room 104. CA Restrooms must be clean and sanitized

Mold was observed inside the ice machines on floors 1, 2, and 3. CA Sanitize equipment to prevent contamination

A black substance was observed around the perimeter of the bathtub inside room 411. CA Recommend recaulking and or resurfacing the wall of the bathtub

Mold was observed inside the ice machines on floors 1, 3, and 4. CA Sanitize equipment to prevent contamination

Stains were observed on the bed bug cover inside room 412 and on the mattress of the let out couch inside room 324. CA Replace and or sanitize bedding and linens to ensure facilities are in good condition

The floor of the showers inside rooms 526 and 324 had a dirt-like appearance. CA Recommend resurfacing the showers. Rust was observed on the wall inside the shower near the shampoo holder inside room 324. CA Reinstall the shampoo holder and remove the rust

The sheetrock material on the ceiling near the front door was observed cracked and peeling. CA Physical facilities must be maintained and in good condition

Mold was observed inside the ice machines on floors 3, 4 and 5. CA Sanitize equipment to prevent contamination

A stain was observed on the mattress pad of the box spring inside room 323. A stain was observed on the pillow case inside room 110 and on the mattresses inside room 532. CA Linen must be clean and in good condition

Soap was not available inside the restrooms of the lobby. CA Provide soap inside the restrooms

Mold was observed inside the ice machines on floors 1, 4, 3 and 5. CA Sanitize equipment to prevent contamination. The ice machine on the first floor was observed leaking and standing was was observed in the area. CA Repair the ice machine to ensure leaks are controlled
