3-1D - required records: shellstock tags, parasite destruction
Regulation: 511-6-1.04(3)(k) - molluscan shellfish, maintaining identity (pf)
Observed no record keeping for shellstock tags for the fresh oysters in the facility. CA: Except as specified under paragraphs 3.(ii) of this subsection, molluscan shellfish tags shall remain attached to the container in which the shellstock are received until the container is empty. The identity of the source of molluscan shellfish that are sold or served shall be maintained by retaining product tags or labels for 90 calendar days from the date that is recorded on the tag label, or invoice as specified under paragraph 2. of this subsection, by: Using an approved record keeping system that keeps the tags, labels, or invoice in chronological order correlated to the date when, or dates during which, the molluscan shellfish are sold or served. PIC will begin keeping a record of the shellstock tags for the fresh oysters.
2-2A - management knowledge, responsibilities, reporting

Observed no employee health policy in the facility. CA: The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee. PIC will have a copy of the employee health policy kept in the facility at all times.
16B - plumbing installed; proper backflow devices

Observed the handwash sink not connected to the drain pipe at the bar area. CA: A plumbing system shall be repaired according to law; P and maintained in good repair. COS PIC reconnected the sink to the pipe.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(6)(r) - single-service/single-use articles, use limitations (c)
Observed a small to go sauce cup being used as a scoop in a large spice container. CA: Single-service and single-use articles may not be reused. COS PIC had small to go sauce container removed.
