Observed lettuce, carrots, and purple cabbage uncovered in the reach in cooler in the front area of the kitchen. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC had an employee to cover lettuce, carrots, and purple cabbage in the reach in cooler in the front area of the kitchen.

Observed cooked stew in a large pot in the walk in cooler uncovered. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC had employee to cover cooked stew in the large pot in the walk in cooler.
4-2B - food-contact surfaces: cleaned & sanitized

Observed brown build up in large ice machine in the front area of the kitchen. Except when dry cleaning methods are used as specified under subsection (7)(e) of this Rule, surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: In equipment such as ice bins. PIC will wipe down large ice bin during shift and will drain, clean and sanitize ice bin at closing today.

Observed hot water not working properly at the employee handwashing sink in the front kitchen area. CA: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC stated that maintenance will be coming today (11/10/2025 to resolve that issue.

Observed no choking poster in the dining area of the facility. CA: A first aid for food choking sign must be posted in the establishment. PIC will be given a copy of a choking poster today to post in the dining area of the facility.
