Repeat Violation: Observed cold holding TCS (Temperature Controlled for Safety) foods above 41°F at Duke Unit on front line. TCS foods must be cold held at 41°F or below. Subway does not use TPHC. See temperature log for said items. COS: All items were discarded. New items from WIC will be put on and ice bath and monitored.

Observed no hand drying provision provided at front handwashing sink. No paper towels available to dry hands after washing. COS: Paper towels were put at front handwashing sink. Employee handwashing sinks must be provided with hand drying provisions.

Observed 6 metal containers of prepared chicken, 4 containers of steak, 4 containers of sliced tomatoes, 2 containers of tuna salad, 4 to 6 containers of chicken, and 4 containers of cold cut combo without dates on them. Containers of food was prepared 04/26/25 to 04/27/25. All TCS foods held over 24 hours must have prepared dates on them. COS: PIC placed dates on all the containers.

Observed several metal containers of cold cut combo 6-meat sitting on the floor in the WIC. All food must be 6" off the floor on a shelf. COS: PIC put all three containers of meat on shelf.

Observed broken/missing floor tiles in the kitchen around three-compartment sink. Will have floor repaired by next inspection date.

Observed TCS foods stored in containers were marked wrong. Steak stored in tuna container and marked 3/18 - 3/21. Chicken stored in steak container. Olives stored in teriyaki container. Steak stored in rotisserie chicken container and marked 3/18 - 3/21. Explained to PIC the violations and he will have all the containers labeled correctly and dated correctly by 03/19. There are several containers marked incorrectly.

Observed soiled food contact surface of Hoshizaki automatic ice machine. Observed black/gray mildew inside food contact surface of ice machine. PIC stated it will be cleared today. Do not serve ice to customers.

Observed soiled nonfood contact surfaces of cabinet under drink machine and wooden shelves in back. The wooden shelves have work paint and they are not easily cleanable. PIC stated everything will be cleaned within three to five days. Also a remodel is taking place in two to three months.

Observed broken/missing floor tile in back area, missing cove molding, walls are soiled/damaged, and soiled floors throughout back area and under/behind shelves and equipment. PIC stated floors and walls will be cleaned within three to five days. Subway is being remodeled in about two to three months.

TCS foods must be cold held at 41°F or below. COS: PIC discarded all of the product. EH discussed using an ice bath under the products to held maintain product temperature and possibly using time as a public health control as well (4 hours).
