Observed container with corn inside reach in cooler and container with juice stored uncovered. COS. items were covered. CA. All foods shall be stored in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination;
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed crock pot with cooked rice with build-up of grease/ food debris on both food and non-food contact surfaces. CA. Equipment food-contact surfaces and utensils shall be clean to sight and touch.
11C - approved thawing methods used

Observed raw chicken being thawed in meat sink at room temperature. COS chicken was placed in reach in cooler. CA. time/temperature control for safety food shall be thawed:

Under refrigeration that maintains the food temperature at 41°F (5°C) or less; or

Completely submerged under running water

Using any procedure if a portion of frozen ready-to-eat food is thawed and prepared for immediate service in response to an individual consumer's order.

Observed tap at meat sink in disrepair. CA. A plumbing system shall be repaired and maintained in good repair. PIC advised to repair tap.
13A - posted: permit/inspection/choking poster/handwashing
Facility has unpaid permit fee.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed heavy build of grease on steam hood, reach in cooler doors, storage rack and fryers. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC advised to clean and maintain equipment.
15C - nonfood-contact surfaces clean

Observed heavy ice crystals inside freezer chests. PIC advised to defrost and clean freezer chests. CA. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed missing ceiling tiles in main kitchen above steam hood. Tiles need to be replaced. Observed floor and walls behind grill and cooking area that are unclean to sight and touch (accumulated grease and food debris). Walls and floor need to be thoroughly cleaned.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(2)(i) - light bulbs, protective shielding (c)
Observed missing light bulb shield in main kitchen. CA. light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food; clean equipment, utensils, and linens; or unwrapped single-service and single-use articles.
