Observed no CFSM documentation posted and PIC unable to provide copy for viewing. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.

No paper towel was available for hand drying at handwash station in main kitchen upon entry. COS paper towel was provided.

Observed open trash receptacle in female bathroom. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

Observed damaged and missing ceiling tiles in main kitchen above ice machine. PIC advised to replace/repair damaged ceiling tiles. CA. All physical facilities shall be maintained in good repair.
