Observed live and/or dead German roaches in all rooms inspected

During the inspection, microwaves, dressers, closet shelves, and/or chairs were observed to be torn or in disrepair in Rooms 142, 320, 329, and 325. These items should be repaired or replaced as necessary to maintain proper room condition and functionality. Observed ceiling light fixtures in disrepair in room 101,142

Loose toilet seats were observed in Rooms 220 and 320. Slow-draining sinks were noted in Rooms 220 and 329. A leak was detected at the sink in Room 142. Evidence of water damage was observed inside the bathroom cabinets in Rooms 132, 220, and 320. A hole in the bathroom ceiling was observed in Room 143. Cracked ceilings and peeling paint were noted in Rooms 132 and 208. Additionally, the bathroom door and baseboard in Room 208 were observed to be in disrepair

Observed gaps/ opening under on or side of doors in rooms 101,237, 307, 220, 320, 132

Observed AC units soiled and/ or in disrepair in all rooms inspected. Observed AC filters soiled and/ or in disrepair in 320, 101, 142, 208, 237, 307, 339

anti-slip protection in the bathtubs/showers was observed to be insufficient in Rooms 101, 142, 208, 237, 307, and 339

During the inspection, four chemical bottles stored on housekeeping carts were observed to be unlabeled at the time of inspection

Observed dead roaches in the following rooms: 102, 137,107 and 305 (occupied room, tenant complained about seeing "baby roaches". CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises

Observed permit 2023 Tourist accommodation permit posted (expired June 2023). Observed that permit fees has not been paid. CA: Informed PIC (person in charge) to pay fees to DeKalb Public health at 445 Winn way, Decatur

Observed stained walls in 236. and copping and the paints on the wall peeling off in the following rooms; 236, 245, and 137. CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed dirty and stained linen in rooms 334, 216, 142, 329

Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. Observed vents dirty and or not working in rooms 334, 309. 211, 140, and 240
