No paper towel was observed at the bathroom sinks and the hand sink on the hallway of the facility. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels; A continuous towel system that supplies the user with a clean toweL. COS: Paper towel was provided.

No hand soap was observed at hand sink in the kitchen. CA: Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. COS: Hand soap was provided.

Observed 3- gallons Borden butter milk expired on 3/13/2026 as of 3/16/2026 (date of inspection). CA: Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. COS: Milk was discarded.

Regulation: 511-6-1.04(6)(e) - cooling methods (pf, c)
Observed beef sambosa cooling at room temperature (67F) on the kitchen counter top. CA: Cooling method can be followed by using one or more of the following methods depending on the type of food being cooled: Placing the food in shallow pans; Separating the food into smaller or thinner portions; Using rapid cooling equipment; Stirring the food in a container placed in an ice water bath or placing the food in the cooler/freezer by loosely covered, or uncovered if protected from overhead contamination. COS: TCS food items was moved from room temperature and placed in the reach in freezer.
12C - wiping cloths: properly used and stored

Observed wet wiping cloth stored on the kitchen counter-top next to the stove. CA: Cloths in-use for wiping counters and other equipment surfaces shall be: Held between uses in a chemical sanitizer solution at a concentration. COS: Wiping cloth was moved and stored inside a chlorine sanitizer water at 50PPM.
18 - insects, rodents, and animals not present
Observed the back door in the main kitchen and back door next to the bathroom with gaps/day light through the door. CA: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Filling or closing holes and other gaps along floors, walls, and ceilings; Closed, tight-fitting windows; and Solid, self-closing, tight-fitting doors.

Observed multiple dead roaches inside the main kitchen oven. CA: Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
