Observed the middle bar, the platinum bar and the B-side bar with out paper towel. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels; A continuous towel system that supplies the user with a clean towel. COS: Paper towel was placed.

Observed the Platinum side bar and the B-side bar without hand soap. CA: Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. COS: Soap was provided for the B-side bar. Person in charge stated that the Platinum bar isn't open frequently.

Observed outer openings with day-lights coming in all exist doors.CA: outer openings of a food service establishment shall be protected against the entry of insects and rodents by:(i) Filling or closing holes and other gaps along floors, walls, and ceilings; Closed, tight-fitting windows; and Solid, self-closing, tight-fitting doors.

Observed the shelves in the walk in cooler to be rusty in appearance. CA: Informed PIC that shelves needs to be replace or resurface. Utensils and food-contact surfaces of equipment shall be made of materials that do not allow the migration of deleterious substances or impart colors and odors.

Observed CFSM not posted in public view.CA: Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM).

Observed both the male and female bathroom door entrance to be open and missing the self closing devices . CA: Except where a toilet room is located outside a food service establishment and does not open directly into the food service establishment, such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door.

Observed the floor underneath equipment to be unclean with food debris. CA: Informed PIC that the physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
