Observed newport cigarettes pack on the counter-top in the main kitchen. CA: Employees shall not use any form of tobacco products, or electronic devices that simulate tobacco smoking, while engaged in food preparation or service, nor while in areas used for equipment or utensil washing and storage, food preparation or food storage. Employees shall only use tobacco products or electronic devices that simulate tobacco smoking in approved designated areas. COS: Item was removed and discarded.
11A - proper cooling methods used: adequate equipment for temperature control

Observed oxtail, curry chicken and brown stew chicken cooling in a tightly closed pan in the two door turbo air reach in cooler.CA: Cooling method can be followed by using one or more of the following methods depending on the type of food being cooled: Placing the food in shallow pans; Separating the food into smaller or thinner portions; Using rapid cooling equipment; Stirring the food in a container placed in an ice water bath or placing the food in the cooler/freezer by loosely covered, or uncovered if protected from overhead contamination. COS: Item was loosely covered to properly.

Observed gnats/flies around the kitchen area. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: Routinely inspecting incoming shipments of food and supplies; Routinely inspecting the premises for evidence of pests. Informed person in charge to get a commercial pest services
14B - utensils, equipment and linens: properly stored, dried, handled

Observed metal dishes stacked while wet stored above the 3-compartment sink. CA: Advised PIC dishes shall be properly air-dried before stacking
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed the gaskets for the turbo air cooler, the G.E mini fridge and the reach in freezers to be unclean. CA: Informed PIC to clean or replace the gaskets in Turbo air cooler, as well as clean all surfaces of food/nonfood contact surfaces.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed broken floor tiles around the kitchen. Observed floors throughout kitchen along perimeter and underneath equipment with food debris and residue. CA: Informed PIC to replace or surface floor tiles around the kitchen area and the physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products
