Observed that the Certified Food safety manager posted is expired (8/31/2025). Informed person in charge to renew CFSM within 90 days. CA: A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment. Within ninety days of the expiration of the CFSM certificate, the CFSM shall enroll in an approved food safety training course, pass an approved exam and obtain a new certificate.

Observed that the current permit fees have not being paid. Informed person in charge to pay permit fees by 1/23/2025.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee phone and airpods on top of food prepping area.CA: Informed person in charge to find a designated area to store personal belongings to prevent contamination. COS: Items was removed and surface was wiped down with bleach sanitizer.
