Observed furniture (desks, chairs, tables, dressers), draperies, appliances (microwave, fridge, AC unit/filters) and/or accessories (lampshades, iron board) unclean and/or damaged in the following rooms: 125, 134, 136, 100, 103, 231, 211, 304, 321, 406

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and maintained in good repair. Observed damaged and or stained bedding in the following rooms: 125, 136, 100, 103, 231, 211, 321, 306

Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. Observed walls/floors/ceiling/fixtures dirty or in disrepair in the following rooms: 134, 100, 103, 231, 211, 304, 406

Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label. Observed 2 sanitizer bottles on a house keeping cart and 2 sanitizer bottles in the laundry room area without labeling indicating the common name of the chemicals

Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Observed doors with inadequate seals in the following rooms: 123, 127, 131, 125, 103, 166, 111, 201, 205, 303

Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Observed live spiders in rooms 125, 103. Observed live and dead roaches in room 200. Observed dead bugs in rooms 407, 408

Observed no stopper in tub and/or sink, tub stained (needs resurfacing), dirty tub, bathroom door and/or door frame in disrepair, vents dirty and/or damaged and/or curtain rod damaged in the following rooms: 125, 132, 103, 115, 200, 201, 324, 406, 407, 408

If a swimming pool or spa at a bed and breakfast inn cannot comply with applicable regulations, then it must be enclosed with a fence at least four feet in height with a locked gate and used only by family members. -Observed pool gate not locked and and with no "Pool Closed" sign posted. COS: The manager got a padlock and and locked the pool gate and posted a "Pool Closed" sign on the gate

Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. -Observed multiple spray bottles of chemical's not labeled with the products common name, located on two different house keeping carts and in the laundry rooms
