3 - hot/cold running water under pressure in all required locations of tourist accommodation
Regulation: 511-6-2-.07(2) - hot water supply
Observed multiple rooms with inadequate water temperatures in multiple rooms. CA: Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall be between 110F and 120F
6 - premises free of pests and rodents; pesticide applied in accordance with laws, rules, and label
Regulation: 511-6-2-.13(1) - insect and rodent control
Observed live and dead roach activity in rooms 236 and 107. The roach activity in room 236 was egregious. Room 107 had dead and live roaches behind the refrigerator. CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed multiple stains and holes on bedding and linens in rooms 233, 220, 107, and 126.
Observed roaches on bed in room 136
24 - openings to outside and physical facilities protected by tight-fitting doors, screening, or other means
Regulation: 511-6-2-.13(3) - insect and rodent control-openings
Observed door to entrance that connects to the outdoors with gaps that give pests access to rooms in room 220, 235, 123, 122, 124, and 235. CA: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Screen doors shall be self-closing and screens for windows, doors, skylights, transoms and other openings to the outside shall be tight-fitting and free of breaks. Screening materials shall not be less than sixteen mesh to the inch. Screen doors for sliding patio doors will not be required to be self closing except in food service, preparation and utensil washing areas of permitted food service establishment kitchens and bed and breakfast inns
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation

Observed stains on the walls in rooms: 236, 233, 220, and 107. CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean
