Observed no hot water at the sinks in the facility. CA: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC will have the water heater repaired.

Observed employee drink stored on the top shelf of the reach in cooler. CA: Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. PIC will store employee items in a designated area where they cannot possibly contaminate facility product.

Observed no paper towels at the handwashing sink in the women and men restrooms. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with individual, disposable towels. PIC will purchase paper towels for the hand washing sinks.

Observed no hot water at the sinks in the facility. CA: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC will have the hot water generation and distribution system repaired to meet peak hot water demands.

Observed no hot water at the hand sink in the men's restroom. CA: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC will have the hot water generation and distribution system repaired to meet peak hot water demands.

Observed no test strips for the three-compartment sink. CA: A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. PIC will provide test strips or the three-compartment sink.

Observed no covered trash receptacle in the women's restroom. CA: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. PIC will provide the women's restroom with a covered trash receptacle.

Observed broken floor tiles in the main kitchen area. CA: Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have the floor tile repaired to be smooth and easily cleanable.

Observed no thermometer used to ensure proper food temperatures in facility. CA: Food temperature measuring devices shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures as specified under DPH Rule 511-6-1-.04. PIC will provide a thermometer for the facility.

Observed no hot water at the hand sink in the men's restroom. CA: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC will have the hot water problem corrected.

Observed the dumpster lid open. CA: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight-fitting lids or doors if kept outside the food service establishment. PIC will close the dumpster lid.

Observed the tile in the kitchen not in good repair. CA: (a) Good Repair. All physical facilities shall be maintained in good repair. PIC will have the tile repaired.

Observed no employee health policy for the facility. CA: (a) Requirement to Report Symptoms, Diagnosis and History of Exposure. The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee: PIC was provided with a copy of the employee health policy.
