Observed the ice machine not clean to sight or touch. CA: Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified by the manufacturer; or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. PIC will have the ice machine interior washed and sanitized.

Observed a missing ceiling tile above the ice machine. CA: Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have the ceiling tile replaced.

Observed no test strips for the sanitizer at the three-compartment sink. CA:A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. PIC will supply test strips for the sanitizer.

Observed business shirts being stored among the equipment, utensils, linens and single-service and single-use articles for the facility. CA: Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. PIC will provide a designated area for the business shirts to be stored.
