The PIC could only provide one signed employee health policy CA: PIC must provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. Must be corrected in 10 days
4-2B - food-contact surfaces: cleaned & sanitized

Observed old food residue remaining on food contact surface of several plates store under grill in cook area and on utensils and silverware in dish washing area of main kitchen. CA: Food contact surfaces must be cleaned to sight and touch. PIC had employees remove and reclean items.

Observed black residue inside ice machine on lid in main kitchen. CA: Food contact surfaces must be cleaned to sight and touch. Employee cleaned ice machine lid

Observed multiple stacks of plastic containers on drying rack in storage room stacked wet. CA: After cleaning and sanitizing, equipment and utensil shall be air-dried. Must be corrected within 10 days
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed accumulation of standing water/residue throughout main kitchen on floors in between and under broken tiles. Observed white caulking material hanging from hood filters above flat grill, residue on shelf holding printer above flat grill and residue on heat lamp above shelf holding foods ready for service to customers. CA: Nonfood-contact surfaces of equipment shall be kept free of residue accumulation
