Observed black light stains on bedding in rooms 115, 213, 217, 226, and 102. (See details at bottom of report). CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be maintained clean

PIC unable to provide proof of gas fire heating inspection. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed stains on linens of rooms 106, 102 and 214. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be maintained clean

Observed chipped paint on door and door frame of rooms 121 and 104, on bathroom door of room 207 and bathroom door frame of room 215. Observed front door of room 228 missing gasket seal at bottom of door. Observed lightly stained walls in room 121. Observed armoir in room 121 and desk in room 207 with stains/deterioration CA: Floors, walls, ceilings, windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed brown/black residue inside ice machine dispenser; CA: In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed chipped paint in bathroom ceiling of room 217, as well as toilet not flushing; also chipped paint along bathroom door frame of room 219; crack/crevice in floor of tub in 220 CA: Physical facility must be maintained and in good repair
