OBSERVED SEVERAL FOODS BEING HELD PAST DISCARD DATE. (E.G. SALSA, SOUR CREAM, COCKTAIL SAUCE, AND ETC.) CA: A food that requires datemarking shall be discarded if it:
(i) Exceeds 7 days, not including the time that the product is frozen; P
(ii) Is in a container or package that does not bear a date or day; P or
(iii) Is inappropriately marked with a date or day that exceeds 7 days . P
- Refrigerated, ready-to-eat, time / temperature control for safety food prepared in a food service establishment and dispensed through a vending machine with an automatic shutoff control shall be discarded if it exceeds 7 days. P

OBSERVED GNATS IN LIQUOR BOTTLES IN THE BAR AREA. CA: Miscellaneous Sources of Contamination. Food shall be protected from contamination that may result from a factor or source not specified under paragraphs (a) through (y) of this subsection.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
OBSERVED CASES OF SODAS STORED DIRECTLY ON THE FLOOR IN THE MAIN KITCHEN. CA: Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food:
(i) In a clean, dry location;
(ii) Where it is not exposed to splash, dust, or other contamination; and
(iii) At least 6 inches (15 cm) above the floor.

Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts.

Pressurized beverage containers, cased food in waterproof containers such as bottles or cans, and milk containers in plastic crates may be stored on a floor that is clean and not exposed to floor moisture.

OBSERVED NUMEROUS LIVE ANS DEAD ROACHES THORUGHOUT THE FACILITY. CA: Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and
-
Eliminating harborage conditions.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(a) - equipment & utensils, air-drying required (c)
OBSERVED SHELVING USED FOR AIR DRYING UTENSILS AND EQUIPMENT NOT INSTALLED PROPERLY. CA: Equipment and Utensils, Air-Drying Required. After cleaning and sanitizing, equipment and utensils:

Shall be air-dried or used after adequate draining before contact with food; and

May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
OBSERVED LARGE OPENINGS IN THE MAIN KITCHEN NEAR THE BACK DOOR. CA: Good Repair. All physical facilities shall be maintained in good repair.
17D - adequate ventilation and lighting; designated areas used

OBSERVED NO LIGHT SHIELDING MISSING FROM LIGHT IN TH MAIN KITCHN AREA. CA: Light Bulbs, Protective Shielding.

Except as specified in paragraph 2 of this subsection, light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food; clean equipment, utensils, and linens; or unwrapped single-service and single-use articles.

Shielded, coated, or otherwise shatter-resistant bulbs need not be used in areas used only for storing food in unopened packages, if:
(i) The integrity of the packages cannot be affected by broken glass falling onto them; and
(ii) The packages are capable of being cleaned of debris from broken bulbs before the packages are opened.
- An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
