Observed hand sink near grill counter to be missing soap and the back hand sink missing paper towels.
RCA: Ensure all handwashing stations are fully stocked and accessible.
10D - food properly labeled; original container
Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed various items throughout facility not in original container without working/common name label ((oil, vinegar, seasonings, mayonnaise, mustard, other sauces).
RCA: Label all items with the common name of the food.
11D - thermometers provided and accurate
Regulation: 511-6-1.05(3)(g) - food tmd, provided and accessible; appropriate tmd for measuring thin foods (pf)
Observed 2 prep coolers with no hanging thermometers in them.
17C - physical facilities installed, maintained, and clean

Observed food on floor near front service counter and debris on the floor in the back of the facility.
RCA: Keep facility maintained and cleaned regularly to eliminate contamination.
