Observed that the facility only has a written disclosure of allergens that specifically says bagels and does not include all of the allergens in the bagels and does not have a written disclosure for the other items on the menu. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.
14A - in-use utensils: properly stored

Observed in-use utensils stored in stagnant, not hot water (86 F). COS. Removed. CA: In-use utensils shall be stored using approved means, such as under running water, in hot (135 F) water, in the product with the handle above the level of the product, or in/on a clean container/surface.
14C - single-use/single-service articles: properly stored, used

Observed single-use items stored upright without overhead protection (straws, trays). CA: Single-use items shall be stored covered or inverted to protect from overhead contamination.
