Observed that the facility did not have an in-date certified food safety manager certification posted and that was not available. CA: Facility shall have a designated food safety manager and that certification shall be available and posted in public view.
2-2B - proper eating, tasting, drinking, or tobacco use
Regulation: 511-6-1.03(5)(k)1&2 - eating, drinking, or using tobacco (c)
Observed employee drink stored in a screw cap bottle that was stored with and above food and food contact surfaces. COS. Removed. CA: Drinks shall be in a single-use cup with a lid and straw and handled to prevent contamination of food and food contact surfaces.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(c) - handwashing sinks, installation (pf, c)
Observed that the hand sink at the dish pit area did not have hot water. CA: Hand sinks shall be equipped to provide water that is at least 85 F.
10D - food properly labeled; original container
Regulation: 511-6-1.04(7)(d) - other forms of information (c)
Observed that the facility did not have written disclosure of the major allergens used in the facility. CA: The facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed that the spray hose at the 3-compartment sink had a leak coming from it. CA: Plumbing system and fixtures shall be maintained in good repair.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed that the inspection report was not posted in the magnolia dining room. CA: The most recent inspection report shall be posted within 15 feet of the main entrance, 5-7 feet from the floor, readable from a distance of 1 foot away, and in a conspicuous place.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed single-use containers stored on the floor of the dry storage room. CA: Single-use items shall be stored at least 6 inches off of the floor.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of derbis on floors; observed damaged ceiling tiles in dining room; observed holes in the walls. CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation; facility shall be maintained in a state of good repair.
