Observed mold or mildew-like substance on the nozzles at the soda machine. CA: Food contact surfaces shall be clean to sight and touch.

Observed leak at 3-compartment sink and the vegetable prep sink had a constant drip from the faucet. CA: Plumbing fixtures and systems shall be maintained in good repair.

Observed that the permit was not posted; the provisional permit was displayed behind the counter where customers could not see it. CA: The food service permit shall be posted in public view.

Observed dumpster did not have a drain plug on their dumpster. CA: Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place.

Observed accumulation of debris on the floor, especially in the walk-in cooler and freezer. CA: Facility shall be clean and cleaning frequency shall be enough to prevent accumulation.

Observed a light out in the food prep area. CA: There shall be adequate lighting in areas where employees are working with food or utensils.
