Observed an employee phone stored on a prep surface while the employee was actively preparing food on that surface. COS. Items removed; surface was washed and sanitized. Code: Food contact surfaces shall be clean to sight and touch.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed employees with hair longer than a half inch that was not properly restrained while they were actively handling food. Code: Hair longer than one half inch shall be properly restrained while working with food.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observed that the back door does not completely close on its own. Code: Exterior openings shall be self-closing and tight-fitting.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(a) - equipment & utensils, air-drying required (c)
Observed containers stacked while wet. Code: Containers shall be thoroughly air dried prior to stacking.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris on floors and walls, particualrly under/behind shelves and behind the pizza oven. Code: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation of debris.
