Observed that the facility was unable to produce verifiable evidence of employee health reporting requirement acknowledgement. CA: Facility shall have verifiable means of showing employee health reporting requirement acknowledgement.
2-2B - proper eating, tasting, drinking, or tobacco use

Observed employee drinks stored in non-single-use cups wihout lids or straws that were stored with and above food and food contact surfaces. COS. Removed. CA: Employee drinks shall be in single-use cups with a lid and a straw and shall be handled to prevent contamination of food and food contact surfaces.

Observed that the facility does not have written disclosure of major allergens used. CA: Facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer

Observed vacuum-sealed fish stored thawed under refrigeration. COS. Removed. CA: Fish stored in reduced-oxygen packaging shall be removed from the packaging prior to or immediately upon completion of thawing.

Observed that the facility was thawing raw chicken by having it sit in stagnant water. COS. Water turned on. CA: Thawing shall be accomplished using approved methods such as under cold, running water, by placing under refrigeration, by microwaving and immediately cooking, or by cooking from frozen.

Observed that there is a large gap under the back door. CA: Exterior openings shall be self-closing and tight-fitting or otherwise protected to protect from pest entry.

Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed that the prep sink and the 3-compartment sink had continuous drips/leaks coming from the fixtures. CA: Plumbing system and fixtures shall be maintained in a state of good repair.

Observed that the facility had an older score of 'A' 95 posted insted of the most recent score of 'B' 89. CA: The most recent inspection report shall be posted within 15 feet of the front door, 5-7 feet from the floor, readable from a distance of 1 foot away, and in a conspicuous place.

Observed containers stacked while wet. CA: Containers shall be thoroughly air dried prior to stacking.
