Observed that the facility does not have written notification of major food allergens used. CA: The facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Due to the unapproved remodel of the upstairs bar, all aspects of the bar including storage and preparation areas are now open and unprotected from entry of pests as there is no self-closing, tight-fitting door or other means of protection. CA: Outer openings of a food service establishment shall be protected against the entry of insects and rodents.

Observed bowls being used as scoops that were stored in the product. COS. Removed. CA: Scoops shall have handles and the handle shall be above the level of the product.

Observed employee belongings stored on and above food and food contact surfaces. CA: Employee belongings and activities shall be in designated areas that prevent contamination.
