Observed raw salmon stored cooked dumplings and ice, raw chicken stored over cooked octopus and an assortment of raw fish, and raw salmon stored over coooked octopus and cooked egg. COS. Coolers rearranged. CA: Food shall be stored in a manner that prevents contamination of food and food contact surfaces.

Observed cold TCS food stored above 41 F (garlic butter, tomatoes, crawfish). COS. To be discarded 4 hours from when it was brought out. CA: TCS food that uses cold holding as a control for safety shall be held at 41 F or below.

Observed hot TCS food stored below 135 F (cooked vegetables). COS. To be discarded 4 hours from when it was brought out. CA: TCS food that uses hot holding as a control for safety shall be held at 135 F or higher.

Observed employee drinks stored in single-cup with only a lid, in screw cap bottles, and in reuseable bottles that were stored with and above food and food contact surfaces. COS. Removed. CA: Employee drinks shall be stored in single-use containers with a lid and a straw and shall be handled to prevent contamination of food and food contact surfaces.

Observed residential pesticides stored in the kitchen. COS. Discarded. CA: All chemicals shall be used according to their conditions of use, including only using residential pesticides in residential settings.

Observed wet wiping cloth bucket on the counter at the sushi bar; observed employee protein powder/workout powder stored on top of refrigerator. COS. Items moved. CA: Chemicals shall be properly stored and separated to prevent contamination of food and food contact surfaces.

Observed many insect traps that had an accumulation of roaches/pests on them. CA: Traps shall be removed at a frequency that prevents accumulation of dead pests.

Observed that the front entrance has a foyer with two doors, however, neither door is tight-fitting. RCA: At least one of the doors needs to be tight-fitting. CA: Exterior openings shall be self-closing and tight-fitting.

Observed that there was a large amount of trash on the ground around the dumpster. CA: Refuse area shall be well maintained.

Observed heavy accumulation of debris, grease, and food throughout the facility, particularly under shelves, equipment, and at the hibachi stations; observed damaged floor tiles and walls throughout the facility. CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation; facility shall be maintained in a state of good repair.

Observed employee belongings stored with and above food and food contact surfaces throughout the facility; phones, backpacks, and jackets throughout. COS. Items moved. CA: Employee belongings and activities shall be in designated areas that prevent contamination of food and food contact surfaces.
