Observed that the facility was unable to produce verifiable means of showing employee health reporting requirement acknowledgement. CA: Facility shall have verifiable means of showing employee health reporting requirement acknowledgement.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed dishes being stored in the hand sink in the back. COS. Emptied. CA: Hand sinks shall be kept clear and accessible and only used for washing hands.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(b) - working containers, common name (pf)
Observed several unlabeled spray bottles. COS. Labeled. CA: Working containers of chemicals shall be labeled with the common name of the product.

Observed that the facility was stored tylenol and tums over the hand sink/prep sink area and icy hot over their frozen foods. COS. Moved. CA: Medications necessary for the health of the employees shall be properly stored to prevent contamination of food and food contact surfaces.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloths stored on counters. COS. Sanitizer solution made and wet wiping cloths placed into them. CA: Wet wiping cloths shall be stored in sanitizer solution at the proper concentration between uses.

Regulation: 511-6-1.05(1)(i) - nonfood-contact surfaces (c)
Observed that the facility using cardboard to line shelves. CA: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed heavy accumulation of grease, debris, and old food on equipment, particularly on and in coolers and shelves. CA: Equipment shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation of debris.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris and grease on floors and walls; particularly behind shelves and equipment. CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.
