Observed a leak under the ice machine and a leaking pipe underneath the 3-compartment sink in the main kitchen. Code: Plumbing fixtures and system shall be maintained in good repair.

Observed facility did not have permit posted. Code: Facility shall have permit posted in a location in the food service establishment that is conspicuous to consumers.

Observed an accumulation of white debris at the bottom of reach in cooler in the back of the establishment. Code: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed accumulation of debris under shelves and equipment as well as small puddles of water from leaking equipment. Code: physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food product.

Observed facility was unable to provide in a verifiable manner, evidence of employee health reporting requirement acknowledgment for all employees; stated that one of the employees is new and they were not able to provide theirs. CA: Facility shall have in a verifiable manner, evidence of employee health reporting requirement acknowledgment.

Observed hand drying provisions were not stocked at the hand sink in the back area. COS. Paper towels placed at the hand sink. CA: Hand sink shall be stocked with hand drying provisions.

Observed employee hair was not effectively restrained; employee had a bandana on, but their hair continued to fall infront of them/over their shoulders and they touched it to move it out of the way several times while working with food. CA: Hair shall be effectively restrained.

Observed employees wearing bracelets. COS. Removed. CA: With the exception of a plain band ring, employees handling food shall not wear anything on hands and wrists to include medical bracelets.

Observed that the fauct at the 3-compartment sink was missing from the stem and that if turned on the water it would just spray, picture attached. CA: Plumbing system and fixtures shall be maintained in a state of good repair.

Observed the facility did not have the permit posted. CA: The permit shall be posted in public view.

Observed that the inspection report was posted upfront, however, it was posted behind another sign (picture attached). CA: The most recent inspection report shall be posted within 15 feet of the primary entrance, 5-7 feet from the floor, readable at a distance of 1 foot away, and in a conspicuous place.

Observed accumulation of debris in equipment, particularly the freezers in the back. CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.

Observed accumulation of debris under shelves and equipment; observed standing water on the floor in the back; the standing water is likely entirely from the mechanical warewasher, the drainage pipe for warewasher shoots into the air like a geyser when draining/emptying (picture and video attached). CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.

Observed employee phone stored on counter and observed employee belongings stored with and above food and food contact surfaces. COS. Removed. CA: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.

Observed the inspection report posted behind the counter and not readable from a distance of 1 foot away. CA: The inspection report shall be posted within 15 feet of the front door, 5-7 feet from the ground, and readable from a distance of 1 foot away in a conspicuous location.

Observed employee were wearing watch and jewelry in the front line. CA: with the exception of a plain band ring no jewelry shall be worn on the hand or wrists while preparing food.

Observed that the inspection report was not posted. CA: Most recent inspection report shall be post within 15 feet of the front door 5-7 feet from the floor.
