Observed that the clean side/sanitizer side of the 3-compartment sink was the dirty side of the mechanical warewasher (clean dishes that were manually washed are being placed by dirty dishes being loaded into the mechanical warewasher. CA: Food contact surfaces shall be clean to sight and touch.

Observed food stored on floor in walk-in cooler. COS. Food moved off floor. CA: Food shall be stored at least 6 inches off the ground.

Observed that the exterior door had a large gap under it. CA: Exterior openings shall be self-closing and tight-fitting or otherwise protected from contamination.

Observed that the facility was stacking dishes while they were wet. CA: Dishes shall be thoroughly air dried prior to stacking.

Observed missing and broken floor tiles throughout the kitchen. CA: Facility shall be maintained in a state of good repair.
