Observed cold TCS items stored below 41 F (cheese, eggs). COS. Items shall be discarded at the end of service. RCA: Discussed requirements for TPHC. CA: TCS food that uses cold holding as a control for safety shall be held at 41 F or below.

Observed that the facility was unable to provide evidence of employee acknowledgement of employee health reporting requirements. CA: Facility shall have verifiable means of showing employee acknowledgement of reporting requirements.

Observed employee drinks stored in non-single use cups. COS. Moved. CA: Employee drinks shall be in single-use cups with a lid and a straw and shall be handled to prevent contamination.

Observed that food was stored in clam shell containers on the counter that are not protected from consumer contamination when open. CA: Food shall always be protected from consumer contamination by means of a sneeze guard, distance, or other similar preventative means.

Observed that the vegetable/fruit sink did not have a splash guard between it and the 3-compartment sink and it shares a faucet with the 3-compartment sink. CA: The vegetable sink shall only be used for washing fruits and vegetables and shall be properly installed to prevent contamination of the produce being washed in it.

Observed that the 3-compartment sink and prep sink only had p-traps. CA: An air gap shall be installed.
