Base violation: Observed raw chicken stored over ground beef and shrimp and raw ground beef stored over raw shrimp in the walk-in cooler; observed raw chicken not in its original packaging stored with and above raw ground beef and raw fish not in the original packaging in the reach-in freezer in the rear of the kitchen. COS. Item organized according to internal cook temperatures. Code: Food shall be stored in a manner that prevents cross-contamination.

Base violation: Observed that the facility does not have written notification of major allergens used. Code: The facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.

Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Base violation: Observed that the squirt bottles on the line were not labeled with the common name of the product stored inside. Code: Non-easily identifiable foods shall be labeled with the common name of the product stored inside.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Base violation: Observed accumulation of food debris on the inside of units (non-food contact surface) at the bar and on the rear prep line. Code: Equipment and non-food contact surfaces shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.

Base violation:Observed employee belongings, such as phones, stored on counters in the front of house area and on the line. COS. Removed. Code: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
