34 - grounds, walkways, porches and hallways maintained; drives provide unobstructed width and proper spacing; graded to drain; trailer spaces marked clearly, areas clean and maintained in good repair
Regulation: 511-6-2-.18(2) - grounds free of litter
Observation: Observed unused mattress, bath tub and other pieces of debris stored beside and behind the facility.
Corrective Action: Discussed keeping grounds free of trash with the PIC. The PIC will have the objects properly disposed of. This will be rechecked by the Health Department
22 - approved leak proof containers covered, in proper location, clean, and maintained; collection adequate
Regulation: 511-6-2-.12(2) - garbage and refuse containers
Observation: Observed garbage dumpster located at facility rusted at the bottom with multiple holes.
Corrective Action: Discussed having the garbage dumpster replaced with the PIC. All outside refuse or garbage storage containers shall be constructed of durable metal or other approved types of materials, which do not leak and do not absorb liquids and shall be provided with tight-fitting lids or covers and shall be kept covered when stored. The PIC will contact the garbage disposal company to order a new dumpster. This will be rechecked by the Health Department
24 - openings to outside and physical facilities protected by tight-fitting doors, screening, or other means
Regulation: 511-6-2-.13(3) - insect and rodent control-openings
Observation: Observed outer opening in rooms 102 and 210 not properly secured when exterior door is closed as evidence by daylight showing. Corrective Action: Discussed replacing weather stripping around doors. Openings to the outside shall be effectively protected against the entrance of pests by tight fitting doors. The PIC will replace the weather stripping. This will be rechecked by the Health Department
