Observation: Observed unused mattress, bath tub and other pieces of debris stored beside and behind the facility.

Corrective Action: Discussed keeping grounds free of trash with the PIC. The PIC will have the objects properly disposed of. This will be rechecked by the Health Department

Observation: Observed garbage dumpster located at facility rusted at the bottom with multiple holes.

Corrective Action: Discussed having the garbage dumpster replaced with the PIC. All outside refuse or garbage storage containers shall be constructed of durable metal or other approved types of materials, which do not leak and do not absorb liquids and shall be provided with tight-fitting lids or covers and shall be kept covered when stored. The PIC will contact the garbage disposal company to order a new dumpster. This will be rechecked by the Health Department

Observation: Observed outer opening in rooms 102 and 210 not properly secured when exterior door is closed as evidence by daylight showing. Corrective Action: Discussed replacing weather stripping around doors. Openings to the outside shall be effectively protected against the entrance of pests by tight fitting doors. The PIC will replace the weather stripping. This will be rechecked by the Health Department

Observed bathroom vent fans with heavy accumulation of dust/ debris in all vacant rooms inspected (Rooms 205, 204, 203, 104). CA: Discussed cleaning methods and frequency of bathroom vent fans with PIC. PIC will have all vent fans properly cleaned. This will be rechecked by the Health Department

Observed multiple washers and dryers no longer in use being stored under stairway in breezeway. Observed broken ice machine (out of service for 6+ months) in breezeway. CA: Discussed keeping grounds free of trash with PIC. PIC will have unused washers/ dryers removed and will have broken ice machine repaired, replaced or removed. This will be rechecked by the Health Department

Observed outer openings in rooms 104, 203 and 206 not properly secured when exterior door is closed. Observed daylight when exterior doors are closed. CA: Discussed replacing weather stripping with PIC. PIC will have weather stripping replaced. This will be rechecked by the Health Department

Observed evidence of rodents in guest rooms 104 and 105. Rodent droppings were observed on and in the linens of room 104 and on the floor and along baseboards of room 105. Corrective Action: PIC was notified. Both rooms were placed out of service. PIC will contact Orkin to inspect and place traps in both rooms. Documentation must be provided for review and rooms must be rechecked prior to renting to the public

Observed broken drawer on dresser in room 203. Observed broken drawer handle on dresser in room 105. Corrective Action: Discussed repairing or replacing furniture in guest rooms when damaged. PIC will repair both dressers before renting these rooms to the public

Observed multiple discarded items, including large mattress and mini fridges stored on the grounds at the end of the building. Observed multiple washers and dryers no longer in use being stored under stairway in breezeway. Observed broken ice machine (out of service for 6+ months) in breezeway. Corrective Action: Discussed keeping grounds free of trash with PIC. This area must be cleaned up and all trash and unused furniture and appliances must be removed by Monday, January 8th. This will be rechecked by the Health Department

Observed multiple discarded appliances and furniture such as chairs, mini fridges, old jet tubs, etc. on grounds at the end of the building.

Corrective Action: Discussed keeping all grounds free of trash with PIC. This area must be cleaned up and all trash and unused furniture and appliances must be removed by Monday, August 12th. This will be rechecked by the Health Department

Observed ice machine with on-going leak but no signage it was out of service. Per PIC, the ice machine does not work.

Corrective Action: Discussed requirements for providing ice with PIC. The facility will have 30-days to have the ice machine repaired or removed. This will be rechecked by the Health Department

Observed filters for AC/Heating unit with dust build up in room 202, 204 and 102. CA: discussed cleaning procedures and frequency of cleaning filters with pic-pic cleaned

Observed dirty air filters for AC units in rooms 103 and 203. CA: PIC will have air filters cleaned
