12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observation: Observed wiping cloth not fully submerged in sanitizer solution when not in use. Observed concentration of chlorine sanitizer solution in chemical sanitizer bucket at greater than 200 ppm.
CA: Wiping cloths must be submerged in chemical sanitizer when not in use. Chlorine chemical sanitizer solution must be kept within acceptable ranges (50ppm-100ppm).
COS: Discussed wiping cloth storage and proper concentrations of chlorine sanitizers with the CFSM. The CFSM remade the sanitizer solution (verified 100 ppm) and will educate staff.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observation: Observed daylight around an exterior door in the main kitchen area when the door was closed.
CA: Outer openings must be protected by tight fitting doors
COS: No COS observed. The CFSM will have the door serviced and or replace the weather stripping around the door. This will be rechecked by the Health Department.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(b) - cutting surfaces (c)
Observation: Observed cutting board on reach in cooler heavily scored/ scratched and unable to be properly cleaned/ sanitized.
CA: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they cannot be resurfaced.
COS: No COS observed. The CFSM will have the cutting board repaired or replaced. This will be rechecked by the Health Department.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observation: Observed gaskets/ seals of multiple coolers in the main kitchen area damaged and in need of replacement.
CA: Equipment components such as gaskets or seals must be kept in good repair.
COS: No COS observed. The CFSM will have the damaged gaskets replaced. This will be rechecked by the Health Department.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observation: Observed residue accumulation on multiple air conditioning vent covers throughout main kitchen and dish room.
CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
COS: No COS observed. The CFSM will have all vent covers properly cleaned so that they are free of residue. This will be rechecked by the Health Department.

Observation: Observed residue accumulation on the nonfood-contact surfaces of multiple reach in coolers in the main kitchen area.
CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
COS: No COS observed. The CFSM will have the coolers cleaned so that they are free of residue accumulation. This will be rechecked by the Health Department.
