Observed open water bottles and a cup without a lid and straw in the dishwashing area along with two 2L soda bottles. COS: PIC was advised to issued employee a disposable cup with lid and straw. PIC directed employee to discard all other items.
Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed.
4-2A - food stored covered

observed shredded carrots in a bucket stored in a RIC without a lid or any covering. COS: PIC was instructed to place a lid or a covering on the container.
(iv) Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination;
4-2B - food-contact surfaces: cleaned & sanitized

Observed quat chemical sanitizer over the concentration limit during the time of the inspection. Bleach sanitizer must be between 50-100ppm. COS. PIC replaced chemical sanitizer. Currently at 200ppm. Advised PIC to limit usage of too much chemical sanitizer must be between 50-100ppm. Corrected On-Site.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed faucets inside of men's & women's restrooms very loose. The sink in the men's restroom falling out of the counter top frame. CA- EHS advised PIC to get repair person to tighten.
Good Repair. All physical facilities shall be maintained in good repair.

observed the light in the dishwashing area failed to have a covering over it. CA: PIC was advised to replace the cover to the light. Good Repair. All physical facilities shall be maintained in good repair.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.05(2)(n) - exhaust ventilation hood systems, designed to prevent grease or condensation from dripping on food/equipment (c)
During routine inspection observed heavy grease deposits stored on ventilation system. Exhaust ventilation hood systems in food preparation and warewashing areas, including components such as hoods, fans, guards, and ducting shall be designed to prevent grease or condensation from draining or dripping onto food, equipment, utensils, linens, and single-service and single-use articles.
