Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles. PIC provided hair restraints.

Observed build-up of slimy pink and black mold substance in the interior of ice machine board. Surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: At any time when contamination may have occurred; at least every 24 hours for iced tea dispensers including nozzles and consumer self-service utensils such as tongs, scoops, or ladles; before restocking consumer self-service equipment and utensils such as condiment dispensers and display containers; and in equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: at a frequency specified by the manufacturer; or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. PIC advised to keep food contact surfaces clean to sight and touch. Ice discard, machine washed and sanitized.

Observed fly activity in the kitchen, storage areas and restroom. The facility must increase fly treatment prior to the next routine inspection. Code- Insect control devices that are used to electrocute or stun flying insects shall be designed to retain the insect within the device or the devices be installed so that they are not located over a food preparation area, and dead insects and insect fragments are prevented from contact with exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles. Facility must take additional steps in conjunction with your pest control contractor such as ensuring no standing water is around the building (especially doors) or debris. Also, keep the dumpster area clean and dry as much as possible. Facility needs better elimination methods for the pests that still make it into the facility.

Observed broken hand sink. All physical facilities shall be maintained in good repair. PIC placed a work order to have if fix within 3 days.
