6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(g) - ready-to-eat time/temperature control for safety food, date marking (pf)
Observed several foods items not date marked- cucumber, lettuce, cheese in the prep cooler located in the front service area. CA: All food items shall be date marked with the discarded and shall be properly dated marked. Person in charge was informed. COS all the concerned food were discarded and dated mark for the one prep on the day of the inspection
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used

Handwashing sink not sealed to the wall
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(a) - floor, walls, & ceilings, cleanability (c)
Observed celling with dust buildup all around the facility specially around the ventilation
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(5)(j) - using dressing rooms & lockers, used for changing clothes and storing employee belongings (c)
Observed personal belonging not properly separated or stored in a designated employees storage shelf or room as required.
