All cold holding and hot holding temperatures in compliance unless otherwise noted. NOTE: An informal inspection will be conducted by 10/10/25. Have all violations corrected by October 10, 2025 or permit may be suspended. ****REQUESTED AN EXTENSION FOR INFORMAL INSPECTION***** EXTENSION GRANTED. 6-1A: .04(6)(f)Observed a large container of cooked chicken and a large container of cooked turkey that were being held higher than 41° F for more than four hours. Also observed a container of fried rice that was cooked the day before that was being held higher than 41° F.// Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P) Corrective Actions: The person in charge discarded the cooked chicken, cooked turkey and the fried rice.//

8-2B: .07(6)(l)Observed two cans of residential use RAID in the kitchen.// Poisonous or toxic materials shall be used according to: (i) Law; (ii) Manufacturer's use directions included in labeling, and, for a pesticide, manufacturer's label instructions that state that use is allowed in a food service establishment, (P) (iii) The conditions of certification, if certification is required, for use of the pest control materials. (P) Corrective Actions: Person in charge discarded the RAID cans.//

12B: .03(5)(g)Observed an employee cooking while wearing a watch.// Food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food (except for a plain ring such as a wedding band). (C) Corrective Actions: Person in charge removed the watch and then washed hands before returning to cooking.//

18: .07(2)(m)Observed that the door to the kitchen does not self close completely. Also observed that the door leading to the outside next to the restrooms does not self close and has large gaps at the threshold where sunlight is visible.SECOND CONSECUTIVE VIOLATION OF THE CODE PROVISION// Outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
- filling or closing holes and other gaps along floors, walls, and ceilings; (C)
- closed, tight-fitting windows; (C) and
- solid self-closing, tight-fitting doors. (C)

- routinely inspection incoming shipments of food and supplies; (C)
- routinely inspecting the premises for evidence of pest; (C)
