Observed expired CFSM certificates posted - no current CFSM for the facility at this time. Within 90 days the CFSM shall enroll in an approved food safety training course, pass an approved exam and obtain a new certificate.

1. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.
2. A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment.
3. All licenses, certificates, diplomas, or other similar credentials issued or granted to an owner or operator who has successfully completed an approved or accredited food safety certification course and exam shall expire on the expiration date determined by the credentialing organization. Within ninety days of the expiration of the CFSM certificate, the CFSM shall enroll in an approved food safety training course, pass an approved exam and obtain a new certificate.
4. The certification is not transferable between persons.

511-6-1.05(6)(n) - Manual and Mechanical Warewashing Equipment, Chemical Sanitization-Temperature, pH, Concentration, Hardness (P,Pf) (n) Manual and Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration, and Hardness. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows:

1. A chlorine solution shall have a minimum temperature based on the concentration and pH of the solution as listed in the following chart; P Note: Observed 3CS sanitizer at a concentration of 10ppm chlorine. A chlorine concentration must be between 50-100ppm. PIC remade sanitizer to 50ppm.

511-6-1.07(4)(c) - Segregation, distressed food segregated and properly located (Pf) (c) Segregation and Location. Products that are held by permit holder for credit, redemption, or returned to the distributor, such as damaged, spoiled, or recalled products, shall be segregated and held in designated areas that are separated from food, equipment, utensils, linens, and single-service and single-use articles. Pf Note: Observed a damaged can in the pantry of facility not segregated from non-damaged cans. Consulted with PIC to have a special area labeled for damaged cans.
