Observed both riser lids to the septic tank to be missing all screws. Observed the riser lid to be easily removed.
Approved Sewage Disposal System. Sewage shall be disposed through an approved facility that is: An individual sewage disposal system that is sized, constructed, maintained, and operated according to law.
Note: PIC will call appropriate personnel to come and have septic riser lids properly screwed on.

Observed several food containers throughout the food establishment to be stored facing upwards instead of an inverted position to prevent contamination.
In a self-draining position that allows air drying; and
(ii) Covered or inverted.
Note: PIC voluntarily inverted all food containers to prevent any possible contamination from occurring.

Observed raw eggs and sauces held on floor in walk in cooler and through out facility. Food shall be protected from contamination by storing the food:(i) In a clean, dry location;(ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. COS: Foods were moved to an approved location and 6 inches above floor.
