Observed expired CFSM posted in the food service establishment. 511-6-1.03(3)(b) - Certification Requirements/Exemptions (Pf) At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals. Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken. Note: The facility will be required to have a food employee with managerial responsibility registered for the exam within 60 days.
2-2B - proper eating, tasting, drinking, or tobacco use

Observed several personal reusable cups next to ice cream mixer equipment in the front food service area. 511-6-1.03(5)(k)1&2 - Eating, Drinking, or Using Tobacco (C) (k) Hygienic Practices. 2. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. COS: The PIC immediately removed the personal cups from food preparation areas.
2-2D - adequate handwashing facilities supplied & accessible

A handwashing facility may not be used for purposes other than handwashing. Pf COS: The PIC cleaned out the sink and instructed employees to only use sink for handwashing.
4-2B - food-contact surfaces: cleaned & sanitized

Observed heavy mold-like accumulation in the ice machine in the main kitchen. 511-6-1.05(7)(b) - Food Contact Surfaces and Utensils - Cleaning Frequency (P, C)(b) Equipment Food-Contact Surfaces and Utensils. 5. Except when dry cleaning methods are used as specified under subsection (7)(e) of this Rule, surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: (iv) In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; or (II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. Note: The facility will need to drain, wash, rinse, and sanitize the ice machine and establish a cleaning schedule necessary to preclude accumulation of soil or mold.

- Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf. COS: The PIC immediately washed, rinse, and sanitized the can opener blade. The blade will need to be replaced as well.
16B - plumbing installed; proper backflow devices

A backflow or backsiphonage prevention device installed on a water supply system shall meet American Society of Sanitary Engineering (A.S.S.E.) standards for construction, installation, maintenance, inspection, and testing for that specific application and type of device. P Note: The facility will need to have this backflow cover replaced to meet American Society of Sanitary Engineering standards.

Observed heavy soil accumulation on gaskets in walk-in coolers and reach-in freezer in the main kitchen. 511-6-1.05(7)(d) - Nonfood-Contact Surfaces (C) (d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Note: The facility will need to have these gaskets cleaned and/or replaced as needed.
17C - physical facilities installed, maintained, and clean

Observed heavy ice build-up in the walk-in freezer on the floor and ceiling with a missing vent cover and a bin full of ice build-up below it. 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C)(5) Maintenance and Operation. (a) Good Repair. All physical facilities shall be maintained in good repair. Note: The facility will need to have the ice build-up removed and the walk-in freezer repaired to prevent ice build-up in the future.
