Observed there to be no employee health agreements in the facility. PIC stated that there were none online or in-store. PIC was unable to provide evidence of alternative training.

Observed the facility's dumpster to be missing a drain plug.

Using Drain Plugs; Maintaining Refuse Areas (C) (o) Using Drain Plugs. Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place.

(p) Maintaining Refuse Areas and Enclosures. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean.

Note: Facility shall replace the drain plug on the dumpster.

Observed leaking 3-compartment sink.

Good Repair. All physical facilities shall be maintained in good repair.

Observed no paper towels at a main kitchen employee hand wash sink. Each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels; A continuous towel system that supplies the user with a clean towel; A heated-air hand drying device; or A hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. COS: PIC placed paper towels at hand wash sink.

Upon entry, observed a hot water pitcher inside hand sink. A handwashing sink shall be maintained so that it is accessible at all times for employee use. COS: PIC was educated on hand sink operations and removed pitcher.
