Observed there to be no employee health agreements in the facility. PIC stated that there were none online or in-store. PIC was unable to provide evidence of alternative training.

Note: EHS provided PIC with a blank copy of the DPH employee health agreement, so that copies could be made and filled out.

Observed the facility's dumpster to be missing a drain plug.
Using Drain Plugs; Maintaining Refuse Areas (C) (o) Using Drain Plugs. Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place.
(p) Maintaining Refuse Areas and Enclosures. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean.
Note: Facility shall replace the drain plug on the dumpster.
