Observed sneeze shields missing from the front serving counter. The sneeze sheilds must be installed by 4/20/26 to prevent potential contamination. Staff members are currently pre assembling multiple trays on top of the hot holding unit shelf.

511-6-1.04(4)(z) - Miscellaneous Sources of Contamination (C) (z) Miscellaneous Sources of Contamination. Food shall be protected from contamination that may result from a factor or source not specified under paragraphs (a) through (y) of this subsection.

Observed the hot water not reaching 85 degrees at two hand sinks. Hot water was also not reaching 85 degrees in the restroom. The maximum temperature that was reached was 79 degrees f. PIC advised to use the sink located on the left side of the kitchen. The hot water reached 90 degrees in that particular sink. PIC will contact maintenance. PIC advised that new hot water heaters were installed. Also observed the water was a brown/yellowish color. PIC advised to run water until it turns clear. A follow up will be conducted on 9/30/25 to check the hot water.

Observed the presence of dead bugs on the floor in the storage area. PIC corrected on site by removing the dead bugs. CA- PIC will monitor the storage area for bug activity. This is a repeat violation. 511-6-1.07(5)(l) - Removing Dead or Trapped Birds, Insects, Rodents, and Other Pest (C) (l) Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Cold water not provided/shut off at employee handwash sink. Observed the cold water not working at the hand sink in the kitchen. PIC will place a work order to have the cold water repaired by 3/10/25.

*** The hand sink in the serving area is not working according to the person in charge. There has been a work order placed with maintenance.
