Observed employee drink with a lid and a straw stored above and next to single service articles (plates) on a cart located behind the serving line. There was also a bag of open chips stored next to the drink. PIC advised that employee food cannot be consumed in the food preparation area. Employee drinks must be stored to prevent contamination. PIC corrected on site by relocating both food and drink items.

Observed a bottle of sanitizer stored above clean dishes in the dish storage area next to the reach in cooler. PIC corrected on site.

Observed the handles on the reach in warmer area broken/loose. The handles must be repaired by 9/28/25 to prevent injury.

Thermometer broken in the walk-in cooler. The thermometer is reading 60 degrees f. The actual temperature was 40 degrees F.

Droppings observed on top of the dishmachine

Condensation leak observed in the reach in cooler. Cooler leaking rusty water on shelves in the cooler (Cooler #2). Cooler not in use at that time. PIC placed an out of order sign on the cooler. Employee cooler also leaking rusty condensation water. PIC relocated employee food to another cooler

Sugar, salt, flour, bin lids observed cracked. Food contact surfaces contained rust build up in the reach in cooler.

Dead water bugs (Roaches) observed in the dish machine area below equipment

Dumpster plug missing

Excessive dust build up observed on the ceiling vents.
