Room 301: Observed staining on carpet in front of bed side table and white staining on couch cushions. Room 211: Observed large and small white staining on couch. Room 217: Observed white staining on couch pillow

Black mold-like accumulation and water damage on ceiling tile in hallway of 3rd floor. Observed pink staining on 2nd floor carpet

Room 202: Observed dust on the furniture and stains on the curtain. Room 302: Observed small stains on the curtain, wear on the desk chair, and stains on the lamp cover. Room 413: Observed small stains on the curtain (oil like), headboards with oil like stains, stain on the closet shelf and burn mark on the desk chair.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Room 202: Observed stains on the couch pillows.

Room 302: Observed big stains on the comforter, mattress, and the couch mattress. Room 302 will need to be closed out until both mattresses have been replaced or cleaned. Please send updates or pictures to sabrina.young@dph.ga.gov. The mattresses must be replaced by 8/8/25. Room 302 cannot be reopened until the cleanliness of the mattresses have been checked by the DPH.

Room 413: Observed stains on the couch pillows, couch mattress, top sheet, and the pillow covers. Also, observed hair on the pillow covers.

Observed the current permit posted is expired and facility owes the Henry County Health Department a total of $225.00. A warning notice was reviewed and signed by PIC. The fee must be paid by 8/7/25

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists

511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice

Observed chemical bottles on housekeeping cart not labeled. Also, several chemicals in the laundry room no label. PIC advised to label all chemical bottles with common name. 511-6-2-.17(2) - Housekeeping Carts Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label

Observed gas fired equipment form not provided. The gas fired equipment form that was provided was date 8/2023. Please have the gas fired equipment inspected and email a copy of the inspection report to robert.lewis1@dph.ga.gov by 1/15/25. 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Room 301-Observed vent not working. 511-6-2-.08(5) - Toilet and Bathing Ventilation Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Stained couch cushions were also observed

Room 316: Observed yellow stain on pillow and pillow cover, staining on pillow cover and mattress, and debris on box spring. -Room 310: Observed stained pillows, mattresses, pillow case and pillow cover, tear in sheets, hair on pillow cover and pillow case, and debris on pillow case. -Room 309: Observed staining on mattress (pull-out), pillow cover, mattresses, and a pillow cover stained with blood. Observed debris on mattress (pull-out), mattress pad and sheets, and hair on pillow cover. -Room 308: Observed stained mattress (pull-out), pillow covers, pillows and mattresses (one heavily stained), and hair in between pillow and pillow covers and on pillow covers, mattress pads and sheets. Also observed tear in pillow cover and sheets. -Room 301: Observed tissue on box spring, stained pillows, pillow cover, mattresses, mattress cover and sheets, and hair on mattress (pull-out), comforter and sheets. Also observed debris on sheets, comforter, and pillow case. -Room 401: Observed stained pillows, pillow case, pillow cover, mattresses, sheets and mattress pad, debris on mattress pad, and hair on sheets and pillow case
