Room 171-Observed live bugs, holes, and stains on the sheets. Room 153- Observed green stains and holes in sheets. Room 276-Observed hair and debris on mattress. Also observed red stains on sheets. Room-154 Mattress cover stained

Observed the annual gas fired equipment report expired (5/16/24). The report must be done annually by a qualified license contractor or local fire authority.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed a heavy mold build up on the draperies in room 169. The curtains must be removed and the room must be treated for mold

In each room all linen had holes, stains, and hair throughout the bedding

Observed several metal baking pans stored under the AC units on the first and second floors of the motel. The AC units need to be repaired due to excessive condensation leaks. Please provide a complete service report showing the AC units have been repaired to sabrina.young@dph.ga.gov

Room 276: Observed stain towels Room 153: Observed the toilet and the shower floor stained. Room 169: Observed the ceiling and the door in bad repair. Also, calcium build up on the walls in the restroom

Observed floors (carpet) in need of a deep cleaning in all rooms inspected except for room 276. There appeared to be a very strong odor in the rooms. PIC advised to have the carpet deep cleaned by 8/5/25

Observed the laundry room walls in bad repair. The walls had black mold build up. The mold will need to be treated and then repainted

Observed chemical bottles (2) not labeled with a common name

Room 161: Observed the dresser and nightstand in bad repair. Observed cigarette stain on the microwave. Room 153: Observed the curtains stained and the top of the dresser in bad repair

Room 165: Observed holes in sheet and the mattress and mattress cover stained. Room 161: Cigarettes holes on the mattress and mattress cover. Also holes on the top sheet. Room 153: Observed the comforter stain and hair and stains on the linen

Room 165: Observed the curtains in bad repair. Observed the baseboards coming off the wall behind the bed. Also, the dresser in bad repair, and the exterior/top of the refrigerator damaged. Room 161: Observed food debris behind the bed. Observed the baseboards damaged underneath the sink. Room 153: Observed socks, hair and trash on the floor behind the bed

In room 171 the guest advised that the light switch is not working properly. Observed a delay

Room 171: Observed hole in comforter, burn mark on sheets, and staining on sheets, pillows and comforter. -Room 173: Observed hair on pillow case, hole in comforter, stain on comforter and heavily stained mattress. -Room 161: Observed stained pillow case, pillow, cover, mattress, and mattress. Comforter was observed with holes in it. -Room 276: Observed heavily stained mattress, comforter with holes and staining, and excessive staining and hair on mattress. Pillow case and sheets contained staining, and the mattress cover contained debris
