Observed water being caught in a metal pan in the reach in cooler. Please put in a work order for the reach in cooler to check to see if a drain line is clogged, if not, then the cooler will need to be repaired.

Observed personal items (unopened candy bar and a notebook) stored in the drawer with clean utensils. Advised PIC to make sure that employees know not to store personal items in the same section with food contact surfaces or areas where food items are stored for the students/staff. All employees items should be stored in a designated area.

Observed a few dead bugs inside of the dishwasher. Even if it is not in use the equipment does require cleaning regularly.

Observed a piece of hair in the ice scoop bin. PIC took the bin down and washed the scoop and bin (COS).

Observed the ice machine chute with a black mold-like substance on the interior top part of the chute. The chute will require special attention for cleaning or a third party company/maintenance team can perform a deep cleaning to ensure that the ice will not be contaminated due to debris. The cleaning of the ice machine started while in the facility. (COS)

Observed chemicals stored with single services items (cups, lids, trays, boxes of gloves). Chemicals will need to be stored in a designated area with chemicals.

Observed spider webs in the ceiling in the dry storage room. Please make sure to dust the ceilings.

Observed the big AC vent along the wall by the walk in cooler in need of cleaning due to a thick dust debris.

Observed the ventilation hood system in the kitchen with a dust debris. Vents will need to be cleaned at least twice a year to prevent contamination.

Observed a new manager of the kitchen. She is awaiting to be registered for the exam, but has not been enrolled yet. School does not have a CFSM on staff in the kitchen. Please obtain ASAP.
