Observed employee prepping pizza with a watch and bracelet on their wrist. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. Educate employees on jewelry wearing procedures. Employee was told to remove their watch and bracelet when prepping food.

Observed that the Certified Food Safety Manager (CFSM) certificate was not posted for public view. The original CFSM certificate shall be posted in public view in each food service establishment. Please post the certificate for public view.

Observed no Certified Food Safety Manager (CFSM) at the facility. The previous CFSM no longer works at the facility. Food service establishments shall employ a Certified Food Safety Manager (CFSM) to ensure food safety is being managed within the establishment during all hours of operation. Have an employee register for an accredited CFSM course and take the exam.

Observed no employee reporting agreenent documents from the employees who are employed at the facility. All employees who work at the facility must understand the employee reporting agreement document, which explains the foodborne illnesses and the symptoms caused by them. Each employee must have the employee reporting agreement document signed and kept on file at the establishment when asked by the health authority. Corrected on Site (COS): The Person in Charge (PIC) read and signed the document, and a copy of the document was left with the PIC to make copies for the other employees to read and sign.
